Please call us at 847-724-7825 to schedule an appointment.
If you need to cancel or reschedule your appointment, please notify our office 24 hours in advance.
If you arrive more than 15 minutes late, every attempt will be made to see your child but you may be required to reschedule the appointment.
Please bring with you any medical records including lab tests and x-ray studies which may be useful.

Physician Availability
If you would like to speak with the physician, please leave a message with the office. Non-urgent matters will be returned within 24 hours and usually by the end of the day. Please let us know if your call requires immediate attention. For emergency issues after-hours the physician is available on page. The doctor usually answers pages immediately, but if she has not returned your page within 30 minutes feel free to page again.
Fees and Billing
- Our fees are based on published local and national averages.
- If required by your insurance company, co-payments will be collected at the time of check-in.
- For fee for service patients, payment is due on the day of service.
- Outstanding balances not paid by indemnity insurance plans will be considered the patients responsibility.
- Balances older than 60 days may be sent to a collection agency.
- Payment may be made by cash, check, or credit card (MasterCard or Visa).
